Welcome to EZ Medical Transportation (“EZ Medical,” “we,” “us,” or “our”). By accessing or using our website, scheduling a ride, or utilizing any of our transportation services, you (“you,” “your,” or “client”) agree to these Terms of Service (“Terms”). Please read them carefully before using our services.

1. Services Provided

EZ Medical Transportation provides non-emergency medical transportation services within Maricopa County, Arizona. Our services include rides to and from medical appointments such as dialysis, surgery, dental visits, physical therapy, pharmacy trips, and other healthcare-related appointments.
We specialize in safe, comfortable, and dignified transportation for individuals of all body types and mobility levels.

2. Eligibility

To use our services, you must:

  • Be at least 18 years old or have a legal guardian schedule on your behalf.

  • Provide accurate and complete information when booking.

  • Be medically stable and not require emergency medical care.

EZ Medical Transportation does not provide emergency medical services. In an emergency, please call 911.

3. Scheduling and Cancellations

  • Rides may be scheduled by phone or online at least 24 hours in advance, unless otherwise arranged.

  • Same-day service may be available depending on driver availability.

  • Cancellations must be made at least 2 hours before the scheduled pickup time.

  • Late cancellations or “no-shows” may incur a cancellation fee.

4. Payment Terms

  • EZ Medical Transportation accepts private pay and provider-scheduled rides.

  • Payment is due at the time of service unless other arrangements are made.

  • Accepted payment methods will be listed on the website or confirmed during booking.

  • We reserve the right to suspend service for unpaid balances.

5. Passenger Responsibilities

To ensure safety and timeliness, passengers (or their caregivers) must:

  • Be ready at the scheduled pickup time and location.

  • Notify EZ Medical of any special needs (e.g., wheelchair, mobility device).

  • Use seatbelts and any safety restraints provided.

  • Refrain from smoking, eating, or consuming alcohol during transport.

  • Treat drivers and staff with respect at all times.

We reserve the right to refuse or discontinue service to anyone who engages in unsafe, abusive, or illegal behavior.

6. Driver Standards

All EZ Medical drivers undergo background checks, defensive driving, CPR, and First Aid training. Our vehicles are regularly inspected and maintained to meet or exceed state and federal safety standards.

7. Privacy and HIPAA Compliance

EZ Medical Transportation complies with the Health Insurance Portability and Accountability Act (HIPAA). Any personal or medical information collected for scheduling or transport is protected and used only for service delivery, billing, or operational purposes.

8. Liability Disclaimer

While we take every precaution to ensure a safe and comfortable experience, EZ Medical Transportation is not responsible for:

  • Delays caused by traffic, weather, or factors beyond our control.

  • Items lost or left in vehicles.

  • Any medical emergencies that arise during transport.

To the fullest extent permitted by law, EZ Medical Transportation disclaims liability for indirect, incidental, or consequential damages arising from service use.

9. Website Use

You agree not to misuse our website by:

  • Attempting to disrupt or hack systems or data.

  • Using the site for fraudulent or unlawful purposes.

  • Submitting false or misleading information.

We may suspend access to the website for maintenance, updates, or violations of these Terms.

10. Modifications

EZ Medical Transportation reserves the right to modify or update these Terms at any time without prior notice. Changes will be posted on this page with an updated effective date. Continued use of our services after updates constitutes your acceptance of the new Terms.

11. Governing Law

These Terms are governed by the laws of the State of Arizona, without regard to its conflict of law principles. Any disputes will be resolved in the state or federal courts located in Maricopa County, Arizona.
