Ride Scheduling Checklist
Before scheduling your ride, please have the following information ready to ensure fast and accurate service:
1. Passenger Information
Full name of the passenger
Date of birth (for identification)
Contact phone number (for passenger or caregiver)
Any mobility or accessibility needs (e.g., wheelchair, walker, lift assistance)
2. Appointment Details
Pick-Up Address: Exact address where the passenger will be picked up
Drop-Off Address: Name and address of the appointment location (doctor’s office, clinic, hospital, etc.)
Appointment Time: The exact start time of the appointment
Requested Pick-Up Time: When you would like the driver to arrive
Return Trip: Indicate if you need a return ride and approximate pickup time
3. Payment Information
Type of payment (Private Pay, Medicaid, or Healthcare Provider-Scheduled)
Accepted payment methods (credit/debit card, insurance, or approved provider billing)
Payment or authorization details if required
4. Special Notes (Optional)
Any medical conditions or assistance the driver should be aware of
Emergency contact name and phone number
5. Scheduling Guidelines
Rides must be scheduled at least 24 hours in advance
Same-day requests may be available based on driver availability
Please be ready 10 minutes before your scheduled pickup time
Cancellations should be made at least 2 hours before your scheduled ride to avoid fees
